Position Title: CPC Population Health VISTA

Start Date: June 3, 2024 (or later in summer 2024)

AmeriCorps VISTA Overview: AmeriCorps VISTA is a one-year volunteer position in which members serve in a capacity-building role at a nonprofit organization or a government agency. Members receive a living allowance (approximately $877 biweekly), healthcare benefits, transportation support, some childcare benefits, professional development & training, and the choice of an education award of $7,395 or cash stipend of $1,800 at the end of the service year. The Center for Primary Care (CPC), located in South Providence, is Rhode Island Hospital’s outpatient primary care and safety net clinic, serving 12,000 patients a year. The CPC serves low-income Rhode Islanders, with more than 80 percent of patients under- or un-insured. The CPC is also home to five “social medicine” programs: the Complex Care Clinic, the Refugee Clinic, the Recovery Clinic, the Transitions Clinic, and the Shower to Empower homeless outreach-medical partnership. The programs, which employ community health workers and provide wraparound services, serve particularly vulnerable Rhode Islanders: individuals with a history of frequent hospitalizations, lack of citizenship documentation, opioid use disorder, incarceration, and homelessness. This VISTA position will continue the work of the previous CPC Care Coordination VISTAs by further building the clinic’s capacity to provide additional supports and resources for these patients, develop new systems to improve clinic operations, and help clinic providers better address patients’ social determinants of health.

The primary focus of this VISTA will be addressing population health, with a focus on how to improve the health of our patient population at scale.

Tasks will include:

  • Building upon the efforts of the previous VISTAs by continuing to develop & manage the Clinic’s community outreach initiatives
  • Planning & coordinating pilot projects with a focus on improving specific clinical outcomes
  • Working in conjunction with the CPC Operations & Management teams and IT to build/enhance the current infrastructure for population level health care maintenance and preventive screenings
  • Researching the impact on population health outcomes of interventions implemented during the course of the year
  • Exploring new funding opportunities to enhance Clinic programming.
