Transitions Clinic

TTC is an interdisciplinary, primary care-based program serving individuals recently released from incarceration in Rhode Island. Located at the Rhode Island Hospital Center for Primary Care, the Transitions Clinic provides medical care and peer-driven reentry support to formerly incarcerated patients with two or more chronic illnesses.

Reentry Collaborative

The RRC is a newly formed collaboration among five community-based organizations, designed to serve people transitioning from prison back to communities in the Providence metro area.

Criminal Justice Policy Collaborative

During summer 2019, the Center convened colleagues from the RI ACLU, the Nonviolence Institute, and the Formerly Incarcerated People’s Union as well as a cross-sector of partners from multiple disciplines to begin developing a long-term vision for community-driven criminal justice reform.

The Center is home to the Overdose and Health Justice in VISTA team which seeks to increase local community capacity to serve individuals impacted by the opioid crisis, with a sub-focus on individuals involved with the criminal justice system.
